Initial Franchise Fee*
The Initial Franchise Fee is $72,000 USD, paid at the time you sign your Franchise Agreement.
This fee provides access to the Nurse Next Door brand, proprietary operating system, proven business model, and the ongoing franchise support network built over more than 20 years in home care.
It also grants you a protected territory, so no other Nurse Next Door Franchise Partner will compete with you in your defined market area.
The Technology Start-up Fee is $8,000 USD. It covers your setup on the Care Services Platform, the system that manages scheduling, attendance monitoring, caregiver coordination, and client records.
It also covers software licenses and the other technology infrastructure required before you can begin serving clients. This fee is paid once at launch.
Your investment includes the Foundations Training Program, an up to five-day program that introduces you to Nurse Next Door's culture, hiring practices, marketing approach, the Care Services Center model, and the brand's Happier Aging philosophy.
Pre-opening branding and launch marketing support is also included and runs approximately $7,000. This combination means you are not left to figure out a go-to-market strategy on your own.
Beyond the fees above, you will need startup capital to cover launch essentials, professional and regulatory costs (such as business registration and licensing), and a working capital reserve.
That reserve is designed to support your operations during the first three to six months while your client base grows.
The estimated startup capital range, including the Technology Start-up Fee and Training & Launch Support, is approximately $47,286–$145,210 USD. The Initial Franchise Fee is not included in this amount.
Once your franchise is operating, you will pay several recurring fees. These are not all fixed-rate; some scale with your revenue, so your actual monthly obligation will depend on your gross sales.
Royalty: 5% of Gross Sales, subject to annual minimum royalties that escalate year over year per the Franchise Agreement. Your monthly obligation is the greater of the percentage of sales or the applicable minimum.
Care Services Center Fee: The greater of 7% of Gross Sales or $300 per month, applicable once the franchise has operated for 12 months and met certain performance thresholds.
Technology Maintenance Fee: $600 per month, flat.
General Brand Fund: 1% of Gross Sales monthly, one of the lowest brand fund rates among leading home care franchise systems.
Local Marketing: You are required to invest the greater of $1,000 or 2% of monthly Gross Sales in local and online marketing, up to a maximum of $2,000 per month.

In Private Duty home care, there are two types of providers of these services.
Non-medical (non-skilled) services: This includes companionship, cooking and cleaning, light housekeeping, personal care (such as grooming) etc.
Medical (skilled) services: This includes all of the non-skilled services plus hands-on, medical services administered by a Registered Nurse or Licensed Practical Nurse (LPN). This could include medication management, post-hospitalization care, wound cleaning and more.
Being able to provide skilled services means the full-spectrum of care. It gives a home care business owner the advantage of being able to service all types of clients, widening your potential base of clientele.
